中药热奄包联合梯度压力弹力袜预防老年髋关节骨折患者术后深静脉血栓及下肢肿胀的回顾性队列研究

摘要:

目的:探讨中药热奄包联合梯度压力弹力袜对老年髋关节骨折患者术后深静脉血栓发生率(DVT)及下肢肿胀的干预效果。方法:采用回顾性队列研究设计,选取2024年1月至2025年1月收治的老年髋关节骨折患者,采用倾向性评分匹配法对患者进行1∶1匹配,匹配后对照组和观察组各34例。对照组实施常规护理联合梯度压力弹力袜,观察组在对照组基础上增加中药热奄包外敷。比较两组术后14 d DVT发生率、下肢肿胀程度、肿胀消退时长、凝血功能指标及D-二聚体水平。采用重复测量方差分析比较两组D-二聚体水平的时间变化趋势,采用多因素Logistic回归分析DVT发生的独立影响因素。结果:观察组术后14 d DVT发生率显著低于对照组(5.88% vs. 26.47%,P=0.021);术后7、14 d的患侧小腿围及大腿围均显著小于对照组(均P<0.05),肿胀消退时长短于对照组(P<0.001);凝血酶原时间(PT)和活化部分凝血活酶时间(APTT)变化量大于对照组,纤维蛋白原(FIB)变化量及D-二聚体水平低于对照组(P<0.05)。重复测量方差分析显示,D-二聚体水平的时间效应(F=42.568,P<0.001)、组间效应(F=28.734,P<0.001)及交互效应(F=6.892,P=0.002)均有统计学意义,组间效应显示术后3、7、14 d观察组D-二聚体水平均显著低于对照组(P<0.05);组内效应显示观察组术后14 d D-二聚体水平与术前比较,差异无统计学意义(P=0.058),而对照组术后14 d D-二聚体水平仍显著高于术前水平(P<0.001),提示观察组凝血功能恢复更快。多因素Logistic回归分析显示,中药热奄包联合干预是DVT发生的独立保护因素(OR=0.21,95%CI: 0.05~0.89,P=0.034)。结论:中药热奄包联合梯度压力弹力袜可有效降低老年髋关节骨折患者术后DVT发生率,改善下肢肿胀,调节凝血功能及D-二聚体水平,且不增加出血风险,具有良好临床应用价值。

Objective: To investigate the intervention effects of Chinese herbal hot compress packets combined with graduated compression stockings on the incidence of postoperative deep vein thrombosis(DVT) and lower extremity swelling in elderly patients with hip fractures. Methods: A retrospective cohort study design was employed. Elderly patients with hip fractures admitted from January 2024 to January 2025 were selected and matched 1:1 using propensity score matching, resulting in 34 patients in each of the control and observation groups. The control group received routine nursing care combined with graduated compression stockings, while the observation group received additional external application of Chinese herbal hot compress packets on the basis of the control group. The two groups were compared in terms of DVT incidence at 14 d postoperatively, degree of lower extremity swelling, duration of swelling resolution, coagulation function parameters, and D-dimer levels. Repeated-measures analysis of variance was used to compare the temporal trends of D-dimer levels between the two groups, and multivariate Logistic regression analysis was performed to identify independent influencing factors for DVT. Results: The incidence of DVT at 14 d postoperatively in the observation group was significantly lower than that in the control group(5.88% vs. 26.47%, P=0.021). At 7 and 14 d postoperatively, the affected-side calf circumference and thigh circumference in the observation group were significantly smaller than those in the control group(all P<0.05), and the duration of swelling resolution was shorter in the observation group(P<0.001). The changes in prothrombin time(PT) and activated partial thromboplastin time(APTT) were greater, while the changes in fibrinogen(FIB) and D-dimer levels were lower in the observation group compared with the control group(P<0.05). Repeated-measures analysis of variance revealed statistically significant time effects(F=42.568, P<0.001), group effects(F=28.734, P<0.001), and interaction effects(F=6.892, P=0.002) for D-dimer levels. Between-group comparisons showed that D-dimer levels in the observation group were significantly lower than those in the control group at 3, 7, and 14 d postoperatively(P<0.05). Within-group comparisons indicated that the D-dimer level at 14 d postoperatively did not differ significantly from the preoperative level in the observation group(P=0.058), whereas it remained significantly higher than the preoperative level in the control group(P<0.001), suggesting faster recovery of coagulation function in the observation group. Multivariate Logistic regression analysis identified the combined intervention of Chinese herbal hot compress packets as an independent protective factor against DVT(OR=0.21, 95%CI 0.05-0.89, P=0.034). Conclusion: The combination of Chinese herbal hot compress packets and graduated compression stockings can effectively reduce the incidence of postoperative DVT, alleviate lower extremity swelling, and regulate coagulation function and D-dimer levels in elderly patients with hip fractures, without increasing bleeding risk, demonstrating promising clinical application value.
