健康体检人群体重调整腰围指数与胆囊结石的相关性分析

摘要:

目的:探讨体重调整腰围指数(WWI)与胆囊结石风险之间的关联。方法:本研究基于2024年1月至12月中国医科大学附属第一医院体检人群数据,依据是否发生胆囊结石将研究对象分为胆囊结石组(2 303例)和对照组(抽取4 606例)。采用多因素Logistic回归模型分析WWI与胆囊结石风险的关系,并通过限制性立方样条(RCS)和分段回归模型探讨其非线性关联与阈值效应。结果:多因素Logistic回归分析显示,在调整多种混杂因素后,WWI每增加1个单位,胆囊结石患病风险增加1.324倍(OR=1.324,95%CI:1.207~1.452,P<0.001)。随着WWI四分位数分组的升高,胆囊结石风险逐步增加(P趋势性值<0.001)。RCS曲线揭示WWI与胆囊结石风险存在非线性关联(P非线性值<0.05)。阈值分析显示,在拐点10.11以下WWI与胆囊结石风险显著正相关(OR=1.62,95%CI:1.34~1.96,P<0.001)。结论:WWI是胆囊结石的独立危险因素,且两者间存在非线性关联。WWI或可作为评估胆囊结石风险的潜在人体测量学指标。

Objective: To investigate the association between weight adjusted waist index(WWI) and the risk of gallstones. Methods: This study was based on the physical examination population data of the First Affiliated Hospital of China Medical University in 2024 from January to December. The subjects were divided into the gallstone group(2 303 cases) and the control group(4 606 cases after sampling), based on the presence or absence of gallstones. Multivariate Logistic regression models were employed to analyze the association between WWI and gallstone risk. Restricted cubic spline(RCS) and segmented regression models were used to explore potential nonlinear relationships and threshold effects. Results: Multivariate Logistic regression analysis showed that after adjusting for multiple confounding factors, for each 1 unit increase in WWI, the risk of gallstones increased by 1.324 times(OR=1.324,95%CI 1.207-1.452,P<0.001).Further analysis indicated that as the quartile groupings of WWI increased, the risk of gallstones formation progressively escalated(Ptrend<0.001). The RCS curve reveals a non-linear relationship between WWI and the risk of gallstones(Pnonlinear<0.05). The threshold analysis inflection revealed that WWI was significantly positively correlated with the risk of gallbladder stones when it was below the inflection point of 10.11(OR=1.62,95%CI 1.34-1.96,P<0.001). Conclusion: WWI is an independent risk factor for gallstones, and there is a non-linear correlation between the WWI and gallstones. WWI may serve as a potential effective anthropometric indicator for assessing the risk of gallstones.
