儿童支原体肺炎气道黏液栓形成影响因素的Meta分析

摘要:

目的: 系统评价儿童支原体肺炎气道黏液栓形成的影响因素。方法: 计算机检索PubMed、Web of Science、中华医学期刊全文数据库、万方数据库、维普网、中国知网。搜集有关儿童支原体肺炎气道黏液栓形成影响因素的研究,检索时间设定为各数据库建库至2025年1月。由两名研究者各自独立完成文献筛选、数据提取和质量评价。使用RevMan 5.4软件进行Meta分析。结果: 共纳入17项研究,包括1 364例黏液栓组患儿和2 584例对照组患儿。Meta分析提示,年龄、急性C反应蛋白(CRP)、中性粒细胞百分比(NE%)、乳酸脱氢酶(LDH)、D二聚体(D-D)、谷丙转氨酶(ALT)、热程延长、胸腔积液及肺不张是儿童支原体肺炎气道黏液栓形成的危险因素。结论: 儿童肺炎支原体感染后气道黏液栓形成可能涉及炎症反应、组织损伤、凝血功能、并发症等多个层面的共同作用。未来需要数据更完备的研究来进一步探讨其他临床、影像学和实验室结果与儿童支原体肺炎气道黏液栓形成之间的关系。

Objective: To systematically evaluate the influencing factors of airway mucus plug formation in children with mycoplasma pneumoniae pneumonia(MPP). Methods: Databases including PubMed, Web of Science, Chinese Medical Association Journal Database, Wanfang, VIP, and CNKI were searched from inception to Jan. 2025 for studies on influencing factors associated with airway mucus plug formation in pediatric MPP. Two researchers independently screened studies, extracted data, and assessed quality. Meta analysis was performed using RevMan 5.4 software. Results: A total of 17 studies were included, comprising 1 364 children in the mucus plug group and 2 584 in the control group. Meta analysis showed that age, acute-phase C-reactive protein(CRP), percentage of neutrophil(NE%), lactate dehydrogenase(LDH), D-dimer(D-D), alanine aminotransferase(ALT), prolonged fever duration, pleural effusion and atelectasis were risk factors for airway mucus plug formation in children with MPP. Conclusion: The formation of airway mucus plugs in children with MPP may result from the combined effects of inflammatory responses, tissue injury, coagulation dysfunction, and complications. Further studies with more comprehensive data are needed to explore additional clinical, imaging, and laboratory factors associated with mucus plug formation in pediatric MPP.
