COVID-19大流行前后南京地区甲型和乙型流感患儿血常规变化分析

摘要:
目的:探讨COVID-19大流行前后南京地区甲型和乙型流感患儿血常规指标的变化。方法: 采用横断面研究设计,选取2019年和2023年南京医科大学附属儿童医院确诊甲型或乙型流感患儿5 742例,回顾性收集临床数据,对比分析两组患儿血常规指标,并基于年龄和病原体类型进行分层分析。结果:与2019年相比,2023年患儿的淋巴细胞绝对值(LYM)、中性粒细胞绝对值(NEU)、白细胞计数(WBC)、单核细胞绝对值(MON)、C-反应蛋白(CRP)和血小板计数(PLT)均显著下降,而红细胞计数(RBC)和血红蛋白水平(HGB)显著上升。分层分析显示,不同年龄组和病原体感染类型患儿的血常规指标存在差异,但总体趋势同整体分析一致。结论:COVID-19大流行后南京地区甲型和乙型流感患儿的血常规特征发生了变化,既往基于血常规等检验指标建立的流感预测模型可能需要考虑COVID-19的影响。
Objective: To investigate the changes in complete blood count(CBC) parameters among children with influenza A or B before and after the COVID-19 pandemic in Nanjing, China. Methods: A cross-sectional study was conducted by selecting 5 742 pediatric patients diagnosed with influenza A or B at the Affiliated Children's Hospital of Nanjing Medical University in 2019 and 2023. Clinical data were retrospectively collected, and a comparative analysis of CBC parameters between the two groups was performed.Stratified analysis was conducted based on age and type of pathogen. Results: Compared with those in 2019, children with influenza A or B in 2023 showed significantly lower levels of lymphocyte count(LYM), neutrophil count(NEU), white blood cell count(WBC), monocyte count(MON), C-reactive protein(CRP), and platelet count(PLT), while red blood cell count(RBC) and hemoglobin levels(HGB) were significantly higher. Stratified analyses revealed the variations in CBC parameters in different age groups and pathogen types, but the overall trends were consistent with the main findings. Conclusion: The CBC characteristics of children with influenza A or B in Nanjing have changed after the COVID-19 pandemic. Existing prediction models based on CBC parameters may need to take the impact of COVID-19 into consideration.
