微视频宣教联合适应性训练方案对鼻咽癌放疗患者心理应对方式及负性情绪的影响

摘要:

目的:探讨微视频宣教联合适应性训练方案对鼻咽癌放疗患者心理应对方式及负性情绪的影响,旨提高鼻咽癌放疗患者的护理质量。方法:纳入我院2021年1月至2023年2月的126例鼻咽癌放疗患者为调查对象,根据患者入院时间随机分为对照组(n=63)和观察组(n=63)。对照组患者接受常规护理干预措施,观察组患者接受微视频宣教联合适应性训练方案。医护人员采用心理健康状态评价量表(SCL-90)、汉密尔顿抑郁量表(Hamilton Depression Scale, HAMD)、汉密尔顿焦虑量表(Hamilton Anxiety Scale, HAMA)和头颈癌患者生命质量测定量表(QLICP-HN)对鼻咽癌放疗患者心理应对方式情况、负性情绪评分情况和生存质量评分情况进行评价。结果:观察组患者的心理应对方式评分显著低于对照组[(139.52±4.60)分vs.(156.13±5.44)分],差异具有统计学意义(P<0.05)。观察组、对照组患者干预前后抑郁及焦虑评分差值差异具有统计学意义(t=-64.71、-57.77,均P<0.001)。观察组患者心理功能、躯体功能、社会功能、不良反应及共性症状等4个维度的评分均优于对照组患者,差异有统计学意义(t=-28.71、-28.27、-14.45、-28.80, P<0.001)。结论:微视频宣教联合适应性训练方案能够较好地改善鼻咽癌放疗患者的心理应对方式以及负性情绪,经过干预后患者的生存质量得到改善,该联合护理干预模式适合临床推广和应用。

Objective: To explore the effect of micro-video tutorials combined with adaptive training program on the psychological coping styles and negative emotions of nasopharyngeal cancer radiotherapy patients, and to improve the quality of care for these patients.Methods: A total of 126 nasopharyngeal cancer radiotherapy patients admitted to our hospital from January 2021 to February 2023 were included in the study. Patients were randomly divided into a control group(n=63) and an observation group(n=63) based on their admission time. The control group received routine nursing interventions, while the observation group received a combined intervention of micro-video tutorials combined with adaptive training program. Symptom Checklist-90(SCL-90), Hamilton Depression Scale(HAMD), Hamilton Anxiety Scale(HAMA), and Quality of Life in Head and Neck Cancer Patients(QLICP-HN) to assess the psychological coping styles, negative emotions, and quality of life of nasopharyngeal cancer radiotherapy patients.Results: The psychological coping styles score of the observation group(139.52±4.60) was significantly lower than that of the control group(156.13±5.44)(P<0.05). The difference in the difference in depression and anxiety scores before and after the intervention was statistically significant for patients in the observation and control groups(t=-64.71,-57.77, all P<0.001). The scores of the four dimensions of psychological functioning, somatic functioning, social functioning, adverse reactions, and co-morbid symptoms of the patients in the observation group were better than the patients in the control group, and the difference was statistically significant(t=-28.71,-28.27,-14.45,-28.80, all P<0.001).Conclusion: Micro-video tutorial combined with adaptive training program can effectively improve the psychological coping styles and reduce negative emotions in nasopharyngeal cancer radiotherapy patients. After intervention, the patients' quality of life is significantly improved, indicating that this combined nursing intervention model is suitable for clinical promotion and application.
