北京市健康体检人群尿酸对代谢综合征发生的影响研究

摘要:

目的:探讨体检人群尿酸(UA)对代谢综合征(MetS)发生的影响,为MetS的预防提供依据。方法:纳入2018年(基线)和2022年均在北京友谊医院进行健康体检的人群资料,排除基线体检数据符合MetS诊断的人群。根据基线UA水平采用四分位数法分别将男性和女性体检者分为4组,采用Cox比例风险回归模型分析基线UA对MetS发生的影响。结果:共纳入体检人群2 930例,其中男1 366例,女1 564例,中位年龄44(33,55)岁。男性和女性体检人群的MetS发生率均随基线UA水平的增加而相应增加(P<0.05)。女性体检者调整年龄后,以U1为参照组,U2、U3、U4组引起MetS发生风险的OR值分别是1.210(95%CI: 0.720~2.304)、1.728(95%CI:1.073~2.784)、2.177(95%CI:1.366~3.471),差异有统计学意义(P=0.003)。男性体检者随基线UA四分位数的增加发生MetS风险差异无统计学意义(P=0.066)。男性体检者预测MetS的基线UA最佳临界值为323.5 μmol·L-1,女性为253.5 μmol·L-1结论:男性和女性体检人群的MetS发生率均随基线UA水平的增加而相应增加。基线UA水平升高是女性体检者发生MetS的独立危险因素。

Objective: To investigate the effect of uric acid(UA) on the occurrence of metabolic syndrome(MetS) in a population of health checkups and to provide a basis for the prevention of MetS. Methods: Data from people who underwent health checkups at Beijing Friendship Hospital in both 2018(baseline) and 2022 were included, and those whose baseline physical examination data were consistent with the diagnosis of MetS were excluded. Male and female were divided into 4 groups according to the baseline UA level using the quartile method, respectively. The Cox proportional hazards model was used to analyze the effect of baseline UA on the occurrence of MetS. Results: A total of 2 930 participants were included in this study, including 1 366 males and 1 564 females, with a median age of 44(33,55) years. The incidence of MetS increased accordingly with increasing baseline UA levels in both male and female health checkups populations(P<0.05). After adjusting for age in the female health checkups populations, using U1 as the reference group, the ORs of U2, U3, and U4 groups that caused the risk of MetS occurrence were 1.210(95%CI:0.720-2.304), 1.728(95%CI:1.073-2.784), and 2.177(95%CI:1.366-3.471), respectively, and the differences were statistically significant(P=0.003). The difference in risk of MetS with increasing quartiles of baseline UA in male health checkups populations was not statistically significant(P=0.066). The optimal threshold value of baseline UA for predicting MetS was 323.5 μmol·L-1 in male health checkups populations and 253.5 μmol·L-1 in females. Conclusion: The incidence of MetS increased accordingly with the increase in baseline UA levels in both male and female health checkups populations. Elevated baseline UA levels are an independent risk factor for the development of MetS in female.
