睡眠障碍与卒中风险的因果关系:孟德尔随机化研究

摘要:

目的:通过两样本孟德尔随机化(MR)研究方法探究睡眠障碍(SD)与卒中发生之间的潜在因果关系。方法:分别从UK biobank、FinnGen数据库中获取SD和卒中的全基因组关联研究(GWAS)汇总数据,提取显著且独立(P<5×10-8F>10;r2=0.001;kb=10 000)的单核苷酸多态性(SNPs)用于MR分析;采用逆方差加权法(IVW)、加权中值法(WME)和MR-Egger三种方法评估因果关系,并进行各种互补敏感性分析以保证结果的稳固。结果:共筛选到14个SNPs作为工具变量,IVW显示SD与卒中间存在正向因果关系(OR=1.264, 95%CI 1.122~1.423,P<0.05),WME结果与之相似(OR=1.253, 95%CI 1.072~1.465,P<0.05),敏感性分析显示结果稳固无偏倚。结论:睡眠障碍与卒中发生存在正向因果关系。

Objective: To explore the potential causality between sleep disorder and stroke by two sample Mendelian randomization(MR). Methods: The summary data for sleep disorder and stroke were obtained from the genome-wide association study(GWAS) in UK biobank and FinnGen databases respectively. Significant and independent(P<5×10-8;F>10;r2=0.001;kb=10 000) single-nucleotide polymorphisms(SNPs) were extracted for MR analysis. Inverse variance weighted(IVW), weighted median estimator(WME) and MR-Egger were usedto assess causality and various complementary sensitivity analyses were conducted to ensure robust results.Results: A total of 14 SNPs were screened as instrumental variables, and IVW showed a positive causal relationship between SD and stroke(OR=1.264, 95%CI 1.122-1.423, P<0.05), with similar results for WME(OR=1.253, 95%CI 1.072-1.465, P<0.05).Sensitivity analysis showed that the results were robust and unbiased. Conclusion: There is a positive causal relationship between sleep disorder and stroke.
