正念认知对痴呆患者临床干预效果的Meta分析

摘要:

目的: 系统评价正念认知对老年痴呆患者心理健康和生活质量、认知功能的影响。方法: 系统检索中国知网、维普数据库、万方数据库、中国生物医学文献数据库、PubMed、EMbase、Web of Science、The Cochrane Library中与正念认知对老年痴呆患者心理健康和生活质量、认知功能影响相关的试验,检索时限为建库至2023年2月1日。由 2 名研究员根据纳入和排除标准独立进行文献筛选、数据提取并进行质量评价,使用RevMan 5. 4软件进行 Meta 分析。结果: 共纳入13篇文献,包括 748 例病人。Meta分析结果显示,正念认知可以改善抑郁情况(MD=-1.40,95%CI -1.90~-0.90,Z=5.51,P<0.000 01)、减轻压力(MD=-2.73,95%CI -3.44~-2.03,Z=7.62,P<0.000 01),对焦虑(MD=-0.47,95%CI -1.05~0.11,Z=1.58,P=0.12)、生活质量(MD=0.53,95%CI -0.38~1.44,Z=1.14,P=0.25)、认知功能(SMD=1.54,95%CI -1.67~4.74,Z=0.94,P=0.35)无统计学意义。结论: 现有证据表明,与常规护理相比,正念认知在改善患者抑郁、压力方面效果更佳。

Objective: To systematically evaluate the effects of mindfulness cognition on mental health, quality of life and cognitive function of patients with senile dementia. Methods: The tests related to the influence of mindfulness cognition on mental health, quality of life and cognitive function of senile dementia patients were systematically searched on CNKI, Vip, Wanfang, Chinese Biomedical Literature Database, PubMed, EMbase, Web of Science, The Cochrane Library. The search time was from the establishment of the database to October 20, 2022. Two researchers independently performed literature screening, data extraction and quality evaluation according to inclusion and exclusion criteria and used RevMan 5.4 software for meta analysis. Results: Totally 13 articles were included, including 748 patients. Meta analysis results showed that mindfulness cognition could improve depression(MD=-1.40, 95%CI -1.90~-0.90, Z=5.51, P<0.000 1), reduce stress(MD=-2.73, 95%CI -3.44~-2.03, Z=7.62, P<0.000 1), anxiety(MD=-0.47, 95%CI -1.05~0.11, Z=1.58, P=0.12), quality of life(MD=0.53, 95%CI -0.38~1.44, Z=1.14, P=0.25), cognitive function(SMD=1.54, 95%CI -1.67~4.74, Z=0.94, P=0.35) had no statistical significance. Conclusion: The existing evidence shows that mindfulness cognition is better than routine nursing in improving depression and stress of patients.
