青少年抑郁症患者童年创伤经历与健康相关危险行为的关系

摘要：

目的：探讨青少年抑郁症患者童年创伤经历与健康相关危险行为的关系。方法：选取在我院首次就诊的283例青少年抑郁症患者作为研究对象，采用抑郁自评量表(SDS)评估患者的抑郁严重程度，童年创伤问卷(CTQ)评估患者的童年创伤经历，青少年健康相关危险行为问卷(AHRBI)评估患者的健康相关危险行为；对有童年创伤组(CTQ+组)和无创伤组(CTQ-组)患者SDS评分及AHRBI评分进行比较，对CTQ评分与AHRBI评分进行相关分析。结果：283例青少年抑郁症患者中有童年创伤患者97例，占34.28%；CTQ+组患者的SDS评分显著高于CTQ-组(P＜0.05)，AHRBI总分和暴力攻击行为、破坏纪律行为、无保护性行为、自伤自杀行为、吸烟饮酒行为因子分显著高于CTQ-组(P＜0.05)；暴力攻击行为评分与CTQ总分、情感虐待、躯体虐待及性虐待因子分呈正相关(P＜0.05)，破坏纪律行为、吸烟饮酒行为评分与CTQ总分、情感虐待、情感忽视、躯体虐待因子分呈正相关(P＜0.05)，自伤自杀行为评分与CTQ总分及其各因子分均呈正相关(P＜0.05)，AHRBI总分与CTQ总分及各因子分均呈正相关(P＜0.05)。结论：在青少年抑郁症患者中，有童年创伤经历者的比例不低，这类患者抑郁症状更严重，也更容易出现健康相关危险行为。

Objective: To explore the relationship between childhood trauma experiences and health-related risk behaviors in adolescent depression. Methods: 283 cases of adolescent depression treated in our hospital were selected as research subjects. The severity of depression and the childhood trauma experience were evaluated by the scale of depression self-rating scale(SDS) and the scale of childhood trauma questionnaire(CTQ) respectively. The health-related risk behaviors were assessed by the scale of adolescent health-related risk behavior questionnaire(AHRBI). The scores between the childhood trauma group(CTQ+group) and non-trauma group(CTQ-group) were compared and their correlation was analyzed. Results: A total of 283 adolescents with depression were included in this study, including 97 cases of childhood trauma, accounting for 34.28%. The SDS score of patients in CTQ+ group was significantly higher than that in CTQ- group. The total score of AHRBI and the factor scores of violent aggressive behavior, disruptive behavior, unprotected behavior, suicidal behavior, smoking and drinking behavior in CTQ+ group were significantly higher than those in CTQ- group(P＜0.05). The score of violent aggression was positively correlated with the total score of CTQ, the factor scores of emotional abuse, physical abuse and sexual abuse(P＜0.05). The scores of discipline-breaking behavior, smoking and drinking behavior were positively correlated with the total score of CTQ, the factor scores of emotional abuse, emotional neglect and physical abuse(P＜0.05). Suicide behavior score was positively correlated with the total score and the factor scores of CTQ(P＜0.05).The total score of AHRBI was positively correlated with the total score and the factor scores of CTQ(P＜0.05). Conclusion: The proportion of adolescents with depression is higher in those who have childhood trauma experiences. These patients have more severe depressive symptoms and are more likely to have health-related risky behaviors.
