青年女性氧化应激水平与内脏脂肪等级的关系

摘要:

目的:研究青年女性氧化应激水平与内脏脂肪等级(VFG)的关系。方法:共纳入22~35岁青年女性52名,利用人体成分分析仪测量VFG,采集受检者的空腹血清标本,检测抗氧化酶[包括超氧化物歧化酶(SOD)、过氧化氢酶(CAT)、谷胱甘肽(GSH)、谷胱甘肽过氧化物酶(GSH-PX)]和氧化应激产物[包括丙二醛(MDA)、4-羟基壬烯醛(4-HNE)、8-异前列腺素F2α(8-iso-PGF2a)];以VFG中位数(Md=3)分为低等级组(n=37)和高等级组(n=15);采用多因素Logistic回归分析VFG高等级发生的独立危险因素,并绘制受试者工作特征(ROC)曲线,分析各氧化应激指标的预测价值。结果:低等级组的血清GSH水平高于高等级组(P<0.05),MDA水平显著低于高等级组(P=0.01),4-HNE和8-iso-PGF2а水平均低于高等级组(P<0.05),两组的SOD、CAT和GSH-PX水平差异均无统计学意义(P>0.05)。多因素Logistic回归分析结果显示GSH水平(OR=0.064,P<0.05)为VFG高等级发生的保护因素,MDA水平(OR=24.162,P<0.05)为VFG高等级发生的独立危险因素。ROC曲线分析显示GSH水平预测VFG低等级的曲线下面积(AUC)=0.699(95%CI为0.541~0.858),临界值为7.96;MDA水平预测VFG高等级的AUC=0.740(95%CI为0.585~0.894),临界值为5.20。结论:青年女性的氧化应激水平与VFG有关,GSH是VFG高等级发生的保护因素,MDA是VFG高等级发生的独立危险因素,且二者均具有比较高的预测价值。

Objective: We will study the relationship between the levels of oxidative stress and visceral fat grade (VFG) in young women.Methods: A total of 52 young women aged 22~35 years were included.The body composition analyzer was used to measure the VFG,and fasting serum samples of the subjects were collected to test antioxidant enzymes[including Superoxide dismutase (SOD),Catalase (CAT),Glutathione (GSH),Glutathione peroxidase (GSH-PX)]and oxidative stress products[including Malondialdehyde (MDA),4-Hydroxyl Nonenal (4-HNE),8-isoprostaglandin F2α(8-iso-PGF2a)].All subjects were divided into low-grade group (n=37) and high-grade group (n=15) according to the median VFG (Md=3).Multivariate logistic regression was used to analyze the predictive value of each oxidative stress index to the increase of VFG,and the receiver operating characteristic (ROC) curve was drawn to analyze the predictive value of each oxidative stress index.Results: The serum GSH's level of the low-grade group was higher than that of the high-grade group (P<0.05),the MDA's level of the low-grade group was significantly lower than that of the high-grade group (P=0.01),and the 4-HNE and 8-iso-PGF2a's levels of the low-grade group were lower than that of the high-grade group (P<0.05),but there were no statistically significant difference in the levels of SOD,CAT and GSH-PX between the two groups (P>0.05).The results of multivariate logistic regression analysis showed that GSH (OR=0.064,P<0.05) was a protective factor for high-grade VFG,and MDA (OR=24.162,P<0.05) was an independent risk factor for high-grade VFG.The ROC curve showed that the area under the curve (AUC) of GSH's level in predicting VFG low grade=0.699(95%CI=0.541~0.858),and the critical value is 7.96.Meanwhile,the AUC of MDA's level in predicting VFG high grade was 0.740(95%CI=0.585~0.894),and the critical value was 5.20.Conclusion: The level of oxidative stress is related to the grade of visceral fat in young women.GSH is a protective factor for the occurrence of high-grade VFG,and MDA is an independent risk factor for the occurrence of high-grade VFG,and both have relatively high predictive value.
