重症急性胰腺炎凝血功能障碍机制及抗凝治疗的研究进展

摘要:

急性胰腺炎(AP)是一种常见的消化系统疾病,即胆道疾病、大量酒精、暴饮暴食等引起的胰酶激活继以胰腺局部炎症反应为主要特征伴或不伴有其他器官功能改变的疾病。其中20%~25%的病人会进展为重症急性胰腺炎(SAP),病死率高达30%~50%,造成巨大的社会医疗负担。研究表明SAP患者存在不同程度的血液高凝状态或血栓形成,进一步发展为凝血功能障碍。在SAP的治疗中,早期合理应用抗凝药物可明显改善胰腺微循环障碍和炎症反应,同时有利于改善其他重要脏器的灌注,阻止病情恶化,可减少SAP的病死率。因此,抗凝治疗是遏制SAP凝血功能障碍的必要治疗手段。作者对SAP凝血功能障碍机制及抗凝治疗的相关研究进行综述。
