肩关节MR成像评价肩袖损伤易感因素的临床价值

摘要:

目的:评估肩关节撞击综合征与相关肩峰影像学特征之间的联系。方法:采用3.0T MR检查,选取56例肩关节临床异常患者作为观察组,50例无症状志愿者作为对照组,对肩关节MR影像学特征进行评估,所有研究对象均经临床症状及体格检查诊断。结果:两组研究对象肩关节MR影像学特征评估有显著差异(P<0.05):患者肩峰Ⅲ型较对照组多(13∶2),肩峰下距离及肩峰前部覆盖在两组研究对象中均不同[观察组(6.3±1.3) mm,1.18;对照组(8.5±1.6) mm,1.37],肩峰厚度两组亦不同[观察组(8.7±0.7) mm;对照组(7.1±0.8) mm];肩锁关节异常好发于观察组。年龄、性别和肱骨头外侧覆盖两组间差异无统计学意义。此外,肩锁关节退变及肩峰的形状与年龄有关。结论:肩峰—肱骨间容积减小及肩峰弓不稳定均易导致肩袖损伤的发生。
