经鼻高流量氧疗联合肺康复训练对COPD合并呼吸衰竭患者的效果观察

摘要:

目的: 探讨经鼻高流量氧疗联合肺康复训练对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者肺功能、血气分析指标、生活质量的影响。方法: 选择我科2017年1月至2019年3月间收治的COPD合并呼吸衰竭患者60例。采用随机数字表法结合个人意愿将其分为对照组和观察组,各30例。对照组给予持续经鼻低流量氧吸入治疗,观察组给予经鼻高流量氧疗联合肺康复训练治疗。比较两组患者临床症状缓解时间及住院时间,干预12周后用力肺活量(FVC)、第1秒用力呼气量(FEV1)、FEV1/FVC、血氧饱和度(SaO2)、血氧分压(PaO2)、二氧化碳分压(PaCO2)、生存质量改善情况。结果: 观察组患者干预后咳嗽、咳痰、气促等症状缓解时间及住院时间均短于对照组(P<0.05);两组干预前肺功能指标、动脉血气分析指标及StGeorge's呼吸疾病问卷评估表(SGRQ)评分比较,差异均无统计学意义(均P>0.05);干预后观察组FVC、FEV1、FEV1/FVC、6 min步行试验(6MWT)、SaO2、PaO2等指标均升高,差值均大于对照组(均P<0.05),PaCO2、SGRQ评分均降低,差值均大于对照组(均P<0.05)。结论: 经鼻高流量氧疗联合肺康复训练可加速COPD合并呼吸衰竭患者临床症状缓解,缩短住院时间,提高肺功能,改善血气分析指标及生存质量,临床干预效果显著,值得进行应用和推广。
