Th17及Treg在哮喘患儿缓解期和发作期外周血中的表达及其临床意义

摘要:

目的: 探析辅助性T细胞17(Th17)和调节性T细胞(Treg)在哮喘患儿缓解期及发作期外周血中的表达水平及其临床意义。方法: 选取2011年7月至2014年7月唐山市人民医院收治的100例哮喘患儿,依据患病情况将患儿分为发作组和缓解组,并选取同期体检50例健康儿童作为健康组。利用流式细胞仪检测各组受试者外周血Th17、Treg百分比,计算Th17/Treg值;采用荧光实时定量聚合酶链反应法(qRT-PCR)检测研究对象的血清白细胞介素-17(IL-17)、白细胞介素-10(IL-10)水平;测定各组研究对象用力肺活量(FVC)、第1秒用力呼气容积占预计值百分比(FEV1)、峰值呼气流速(PEF)等肺功能指标水平;分析哮喘患儿外周血Th17、Treg百分比及Th17/Treg值与IL-17、IL-10水平及肺功能参数的相关性。结果: 发作组患儿外周血Th17百分比、Th17/Treg值、血清IL-17水平均高于缓解组、健康组(均P<0.05),Treg百分比、IL-10水平及FVC、FEV1、PEF均低于缓解组、健康组(均P<0.05);缓解组患儿外周血Th17百分比、Th17/Treg值、血清IL-17水平均高于健康组(P<0.05),Treg百分比、IL-10水平及FVC、FEV1、PEF均低于健康组(P<0.05);哮喘患儿外周血Th17百分比、Th17/Treg值与IL-17水平呈正相关(P<0.05),Treg百分比与血清IL-10水平及FVC、FEV1、PEF均呈正相关(P<0.05);哮喘患儿外周血Th17百分比、Th17/Treg值与FVC、FEV1、PEF,Th17/Treg值与血清IL-10水平均呈负相关(P<0.05)。结论: Th17/Treg在哮喘患儿发作期和缓解期中呈高水平表达,并随病情加重而升高,对评估哮喘患儿病情有参考价值。

Objective: To explore the level and clinical significance of helper T cell 17(Th17) and regulatory T cell(Treg) in peripheral blood of asthmatic children in remission stage and attack stage.Methods: 100 asthmatic children admitted to Tangshan People's Hospital from July 2011 to July 2014 were selected, according to their condition, the children were divided into attack group and remission group, and 50 healthy children in the same period were selected as healthy group. The percentage of Th17 and Treg in peripheral blood were measured by flow cytometry, and Th17/Treg value was calculated; the levels of serum interleukin-17(IL-17) and interleukin-10(IL-10) in all subjects were detected by quantitative real-time polymerase chain reaction(qRT-PCR); the lung function indexes of forced vital capacity(FVC), forced expiratory volume in the first second(FEV1) and peak expiratory flow rate(PEF) in all subjects were measured; and the correlations between levels of Th17, Treg, Th17/Treg valuein peripheral blood of asthmatic children and levels of IL-17, IL-10, pulmonary function parameters were analyzed. Results: Compared with those in remission group and healthy group, the percentage of Th17, Th17/Treg valuein peripheral blood and the level of serum IL-17 in asthmatic attack group were significantly higher(P<0.05), and the percentage of Treg, the level of IL-10, FVC, FEV1 and PEF were significantly lower(P<0.05); compared with healthy group, the percentage of Th17, Th17/Treg value in peripheral blood and the level of serum IL-17 in the remission group were significantly higher(P<0.05), and the percentage of Treg, the level of IL-10, FVC, FEV1 and PEF were significantly lower(P<0.05). The percentage of Th17 and Th17/Treg value were positively correlated with the level of IL-17(P<0.05), and the percentage of Treg was positively correlated with the level of serum IL-10, FVC, FEV1 and PEF(P<0.05). There were negative correlations between the percentage of Th17, Th17/Treg value in peripheral blood and FVC, FEV1,PEF(P<0.05).Th17/Treg value and the level of serum IL-10 showed negative correlation(P<0.05). Conclusions: Th17/Treg value has a high level in asthmatic children both attackand remission period, which increases with the aggravation of the disease. It is helpful for evaluating the condition of asthmatic children.
