目的：观察老年冠心病（CHD）患者血清补体C1q/肿瘤坏死因子相关蛋白6（CTRP6）水平变化及其与冠状动脉病变程度的关系。方法：选择2015年1月至2017年7月在我院接受冠状动脉造影（CAG）检查的495 例老年患者纳入研究，包括CHD组361 例和CAG阴性（对照）组134 例。采用酶联免疫吸附法（ELISA）检测血清 CTRP6水平，比较CHD组和对照组血清CTRP6水平，观察血清CTRP6水平与老年CHD患者冠状动脉病变严重程度的关系。结果：CHD组和对照组血清CTRP6水平分别为（22.81±7.55） ng·ml^-1和（48.65±8.38） ng·ml^-1，CHD组低于对照组，差异具有统计学意义（t=32.82，P<0.001）。Spearman直线相关分析示血清CTRP6水平与冠状动脉病变数目及Gensini评分均呈负相关（r=-0.25，P<0.001；r=-0.44，P<0.001）。多元线性回归分析示血清CTRP6水平升高是老年CHD患者Gensini评分重度病变的独立保护因素（OR=0.66；95%CI 0.45~0.96；P=0.002）。受试者工作特征（ROC）曲线分析示血清CTRP6预测老年CHD患者Gensini评分重度病变的曲线下面积为0.84。结论：老年CHD患者血清CTRP6水平与其冠状动脉病变程度呈负相关，有望成为老年CHD患者早期诊断及病变严重程度评估的新指标。
