小儿喉罩不同头颈位置对气道密封压和通气效果的影响研究

摘要:

目的: 探究小儿喉罩不同头颈位置对气道密封压和通气效果的影响。方法: 回顾性分析2017年9月~2018年3月期间在我院接受治疗的行全身麻醉的128例患儿,依据喉罩头颈位置分为对照组和研究组,各64例,对照组患儿采取头颈俯屈位,研究组患儿采取头颈正中位,分别在头颈俯屈位、正中位记录患儿的气道密封压、通气效果以及患儿出现的不良反应情况。结果: 对照组患儿的气道密封压较高(P<0.05);对照组的通气效果和通气治疗有效率低于研究组(P<0.05);研究组患儿出现恶心呕吐、屏气、呛咳、出血以及支气管痉挛等不良反应的情况少于对照组(P<0.05)。结论: 小儿喉罩全麻时不同的头颈位置,对气道密封压和通气效果影响不同。

Objective: To explore the effect ofultrasound-guided laryngeal mask anesthesia on airway sealing pressure and ventilation in different head and neck positions of children. Methods: A retrospective analysis of 128 children undergoing general anesthesia in our hospital from September 2017 to March 2018 was conducted. According to the different head and neck positions of the laryngeal mask anesthesia, patients wereequally divided into the control group and the study group, 64 cases in each group.In the control group, the head and neck were bent, and in the study group, the head and neck were median under the guidance of B-ultrasound. The airway sealing pressure, the DBP (diastolic blood pressure), SBP (systolic pressure), HR (heart rate) and SpO2 (oxygen saturation) indicators, the ventilation effects and adverse reactions before and after induction of anesthesiawere recorded.Results: The airway seal pressure was higher in the control group (P<0.05); the effective rate of ventilation and ventilation treatment in the control group was lower than that in the study group (P<0.05); the adverse reactions such as nausea and vomiting, breath holding, choking cough, bleeding and bronchospasm in the study group were lower than those in the control group (P<0.05). Conclusion: Different head and neck positions in general anesthesia have different effect on airway seal pressure and ventilation.
