TVCDS在评估宫颈癌发展程度及疾病分期中的价值及其与Egr-1表达的关系研究

摘要:

目的: 研究经阴道彩色多普勒超声(TVCDS)在评估宫颈癌发展程度及疾病分期中的价值及其与早期生长因子(Egr-1)表达的关系。方法: 本研究将2013年11月-2015年11月本院收治的84例宫颈癌患者作为研究对象,根据宫颈癌病理学分级标准,Ⅰ期患者19例,Ⅱ期患者53例,Ⅲ期患者12例,分析不同病理分级患者的基础资料、TVCDS检查结果、患者的血清Egr-1 RNA之间的差异,同时,对宫颈癌患者不同分级的危险因素进行logistic回归分析,TVCDS检查结果与血清Egr-1的相关性分析。方法: 不同病理学分级的淋巴结转移、病灶大小、脉管累及情况、宫旁浸润情况以及间质浸润深度之间的差异存在统计学意义;淋巴结转移、病灶大小、脉管累及、宫旁浸润、间质浸润深度均为宫颈癌患者的独立危险因素;不同病理分级患者的血流分级情况、血流最大速度(PSV)以及阻力指数(RI)之间的差异存在统计学意义;病理分期Ⅰ、Ⅱ、Ⅲ期患者的血清Egr-1 RNA分别是2.34±0.12、4.45±1.33、7.32±1.92,三组患者的血清Egr-1 RNA之间的差异存在统计学意义(F=12.897,P=0.000);患者的血清Egr-1 RNA分别与患者的TVCDS检查结果中的血流分级、PSV以及RI呈现正相关,差异存在统计学意义(P<0.05)。方法: TVCDS检查对宫颈癌的诊断具有积极的作用,同时TVCDS检查结果中的血流分级、PSV、RI均与血液Egr-1检查结果呈现正相关,对患者的预后判断具有重要意义。

Objective: To study the value of TVCDS in evaluating cervical cancer development and disease stages and its relationship with Egr-1 expression. Methods: In this study, 84 patients with cervical cancer admitted to our hospital between November 2013 and November 2015 were studied. According to the pathological grading criteria of cervical cancer, 19 patients with grade Ⅰ, 53 patients with grade Ⅱ and 12 patients with grade Ⅲ were classified. The basic data of patients with different pathological grading, the results of TVCDS, and the difference of serum Egr-1 RNA were analyzed. At the same time, logistic regression analysis of risk factors in different grades of cervical cancer, correlation analysis of TVCDS results and serum Egr-1 were conducted. Results: There were significant differences in lymph node metastasis, lesion size, vascular involvement, parauterine infiltration and interstitial infiltration depth among different pathological grades. Lymph node metastasis, lesion size, vascular involvement, parauterine infiltration and interstitial infiltration depth were independent risk factors for cervical cancer patients; different pathological grades were associated with cervical cancer. There were significant differences in blood flow grading, PSV and RI in patients with pathological stage Ⅰ, Ⅱ and Ⅲ. Serum Egr-1 RNA in patients with pathological stage Ⅰ, Ⅱ and Ⅲ were2.34±0.12, 4.45±1.33, 7.32±1.92. There were significant differences in serum Egr-1 RNA among the three groups (F=12.897, P=0.000). Serum Egr-1 RNA was positively correlated with blood flow grading, PSV and RI in patients with TVCDS (P<0.05). Conclusion: TVCDS plays an active role in the diagnosis of cervical cancer. At the same time, blood flow grading, PSV and RI in TVCDS results are positively correlated with blood Egr-1 results, which is of great significance for the prognosis of patients.
