血清正五聚素蛋白-3在类风湿关节炎患者中的表达及其临床意义

摘要:

目的:探讨正五聚素蛋白-3(PTX3)在类风湿关节炎(RA)患者血清中的水平及其临床意义。方法:选择2015年1月至2017年12月入住榆林市第二医院的58例RA患者(RA组),并同期选取体检中心年龄、性别相匹配的58例健康体检者作为对照组。收集研究对象的一般人口学数据和临床资料并留取血清,运用酶联免疫吸附试验检测血清PTX3水平,对PTX3水平与实验室参数进行相关性分析。结果:RA组血清PTX3水平显著高于对照组(P<0.05)。此外,活动期RA患者的血清PTX3水平显著高于非活动期患者(P<0.05)。相关性分析结果显示,血清PTX3水平与超敏-C反应蛋白(Hs-CRP)和疾病活动指数28(DAS28)呈正相关(r值分别为0.400、0.372,均P<0.05),但与红细胞沉降率(ESR)无相关性(P>0.05)。ROC曲线分析结果显示PTX3诊断RA的敏感性、特异性、曲线下面积(AUC)分别为75.9%、78.9%、0.849;PTX3区分RA活动期与非活动期的敏感性、特异性、AUC分别为64.5%、74.1%、0.731。结论:RA患者血清PTX3水平显著增高,且其与疾病活动期相关。PTX3可作为RA的一个新型生物标志物。

Objective: To evaluate the expression and clinical significance of serum pentraxin-3 (PTX3) in patients with rheumatoid arthritis (RA). Methods: Consecutive selection 58 RA patients and 58 age- and gender- matched healthy controls (HC) from the Second Hospital of Yulin from January 2015 to December 2017 were enrolled in our study. Demographic data, clinical data and serum of RA patients were collected. The levels of serum PTX3 were measured by ELISA. Correlation analysis between serum PTX3 levels and laboratory parameters was also performed. Results: The levels of serum PTX3 were significantly increased in RA patients compared to HC (P<0.05). In addition, the levels of serum PTX3 were significantly higher in active RA patients than those in inactive stage (P<0.05). Correlation analysis showed that the levels of serum PTX3 were positively correlated with hypersensitive C-reactive protein (Hs-CRP) and DAS28 (r value was 0.400, 0.372, respectively, all P<0.05), but not correlated with erythrocyte sedimentation rate (ESR) (P>0.05). Furthermore, the results of ROC curve analysis showed that PTX3 in the diagnosis of RA, the sensitivity and specificity, and the area under curve (AUC) were 77.9%, 74.0% and 0.797, respectively. The sensitivity and specificity of PTX3 in distinguishing active and inactive RA were 64.5%, 74.1%, and the AUC was 0.731%, respectively. Conclusion: The levels of serum PTX3, associated with the disease activity, are significantly higher in patients with RA. PTX3 can be used as a novel biomarker for RA.
