目的：探讨非酒精性脂肪肝（NAFLD）对老年2型糖尿病患者心脏结构和功能的影响。方法：收集2015年1月至2017年12月在我院老年科住院年龄≥ 60岁的2型糖尿病患者702例，根据腹部彩超结果分为无NAFLD组489例和NAFLD组213例，比较两组间超声心动图各指标的差异。结果：与无NAFLD组相比，NAFLD组患者室间隔及左心室后壁厚度明显增厚，左心室质量明显增大，分别为（0.98±0.01）vs（0.91±0.01）cm、（0.96±0.01）vs（0.89±0.01）cm、（176.62±2.58）vs（159.75±1.69）g，经多变量校正后差异仍具有统计学意义（P<0.05）；与无NAFLD组相比，NAFLD组E波峰值流速（E）与A波峰值流速（A）的比（E/A）值<1的发生率显著升高；二元Logistic回归分析显示，NAFLD是心脏舒张功能障碍的独立危险因素。结论：老年2型糖尿病患者中NAFLD与室间隔厚度、左心室后壁厚度、左心室质量及心脏舒张功能障碍的患病风险呈显著正相关。
