血清长链非编码RNA PVT1联合CA125与CEA在子宫内膜癌中的临床价值研究

摘要:

目的:探究长链非编码RNA(lncRNA)PVT1联合癌胚抗原(CEA)、糖链抗125(CA125)对子宫内膜癌的诊断价值及三者与子宫内膜癌临床病理特征之间的关系。方法:收集2014年1月至2016年1月在我院经病理学/细胞学确诊的子宫内膜癌患者120例与同期在我院行健康体检的健康人群120例。采用实时荧光定量PCR(RT-PCR)法检测受试者外周血中lncRNA PVT1;血清CA15-3和CEA检测分别采用乳胶增强免疫透射比浊法和化学发光法。结果:子宫内膜癌患者lncRNA PVT1、CEA和CA125水平显著高于健康人群,且差异具有统计学意义(P<0.05)。LncRNA PVT1联合CEA和CA125在区分子宫内膜癌与健康人群的曲线下面积(AUC)为0.887(95%CI:0.822~0.951,P<0.001),灵敏度为87.2%,特异度为93.2%。此外,子宫内膜癌患者lncRNA PVT1、CEA和CA125水平与肿瘤TNM分期显著相关(P<0.05);lncRNA PVT1和CEA与子宫内膜癌患者的组织学类型相关(P<0.05),且lncRNA PVT1和CEA水平在透明细胞性腺癌患者中的增高最为显著;而三者与患者性别、年龄等因素无关(P>0.05)。多因素分析显示,高lncRNA PVT1、CEA和CA125水平是子宫内膜癌发病的独立危险因素,对子宫内膜癌发病风险的预测具有指导意义。结论:血清lncRNA PVT1、CEA和CA125对子宫内膜癌具有诊断价值,且与子宫内膜癌的TNM分期、组织学类型显著相关。此外,lncRNA PVT1、CEA和CA125亦可作为子宫内膜癌发病风险预测的参考指标。

Objective:To explore the diagnostic values of long non-coding RNA (lncRNA) PVT1, carbohydrate antigen 125 (CA125) and carcinoembryonic antigen (CEA) for endometrial carcinoma, and the relationship between the three and the clinicopathological features of endometrial carcinoma.Methods:A total of 120 cases of endometrial carcinoma diagnosed in our hospital from January 2014 to January 2016 were collected, and 120 healthy people were recruited in our hospital during the same period. The levels of lncRNA PVT1 in the peripheral blood of subjects were detected by real-time quantitative PCR (RT-PCR). Serum CA15-3 and CEA were detected by latex enhanced immunoturbidimetry and chemiluminescence, respectively.Results:The levels of lncRNA PVT1, CEA and CA125 in endometrial cancer patients were significantly higher than those in the healthy subjects, and the difference was statistically significant (P<0.05). LncRNA PVT1 combined with CEA and CA125 had high value in distinguishing endometrial carcinoma from healthy people, and the area under the curve (AUC) was 0.887 (95% CI:0.822-0.951, P< 0.001) with a sensitivity of 87.2% and a specificity of 93.2%. In addition, the levels of lncRNA PVT1, CEA and CA125 in endometrial carcinoma were significantly correlated with tumor stage of TNM (P<0.05), and levels of LncRNA PVT1 and CEA were associated with histological type of endometrial carcinoma(P<0.05), and the levels of lncRNA PVT1 and CEA increased most significantly in patients with clear cell adenocarcinoma. The three indexes were not related to sex, age and other factors (P>0.05). Multifactor analysis showed that high lncRNA PVT1, CEA and CA125 levels were all independent risk factors for endometrial cancer and of guiding significance for the prediction of the risk of endometrial cancer.Conclusion:Serum lncRNA PVT1, CEA and CA125 have diagnostic value for endometrial carcinoma, and are significantly correlated with the TNM stages and histological types of endometrial carcinoma. LncRNA PVT1, CEA and CA125 can also be used as reference indexes for predicting the risk of endometrial cancer.
