联合检测糖化血红蛋白、胱抑素C、血清淀粉样蛋白A、视黄醇结合蛋白对早期糖尿病肾病的诊断价值

摘要:

目的:探讨联合检测糖化血红蛋白(HbA1c)、胱抑素C(CysC)、血清淀粉样蛋白A(SAA)、视黄醇结合蛋白(RBP)在早期糖尿病肾病(DN)中的诊断价值。方法:选取64例早期DN患者(早期DN组)及60例单纯糖尿病患者(对照组),检测两组HbA1c、CysC、SAA、RBP水平并进行比较。结果:早期DN组HbA1c、CysC、SAA、RBP水平分别为(8.55±1.63)%、(1.68±0.35)mg·L-1、(11.28±2.84)mg·L-1、(86.45±9.74)mg·L-1,均明显高于对照组(P<0.05);早期DN组HbA1c、CysC、SAA、RBP及4项联合的阳性检出率为78.13%、70.31%、62.50%、67.19%及96.88%,均明显高于对照组(P<0.05),且早期DN组4项指标联合检测阳性检出率明显高于各单项指标检出率(P<0.05);早期DN组4项指标联合检测的诊断灵敏度、准确度为96.88%、92.74%,均明显高于各单项指标(P<0.05)。结论:HbA1c、CysC、SAA、RBP与DN密切相关,联合检测对早期DN的诊断具有重要意义。

Objective:To explore the diagnostic value of combined detection of glycosylated hemoglobin (HbA1c), cystatin C(CysC), serum amyloid A(SAA) and retinol binding protein(RBP) in early diabetic nephropathy(DN). Methods:64 patients with early-stage DN(early DN group) and 60 patients with diabetes (control group) were selected. The levels of HbA1c, CysC, SAA, and RBP in the two groups were determined and compared. Results:The levels of HbA1c, CysC, SAA and RBP in early DN group were (8.55±1.63)%,(1.68±0.35)mg·L-1,(11.28±2.84)mg·L-1,(86.45±9.74)mg·L-1, respectively, which were significantly higher than those in the control group(P<0.05). The positive detection rates of HbA1c, CysC, SAA,RBP and the combination of four indicators in early DN group were 78.13%,70.31%,62.50%,67.19%,96.88%, which were significantly higher than those in the control group (P<0.05), and the positive detection rate of the combination of four indicators was significantly higher than that of the individual indicator in early DN group (P<0.05). The sensitivity and accuracy of the joint detection of the four indicators in early DN group were 96.88% and 92.74% respectively, which were significantly higher than those of the individual indicator (P<0.05). Conclusion:HbA1c, CysC, SAA, RBP are closely related to diabetic nephropathy, and joint detection is of great significance in the diagnosis of early renal damage.
