99mTc-tetrofosmin SPECT肿瘤显像在鼻咽癌治疗效果评价中的价值

摘要:

目的:研究99mTc-tetrofosmin (99mTc-TF)单光子发射计算机断层成像(single-photon emission computed tomography,SPECT)肿瘤显像用于鼻咽癌治疗效果评价的可行性。方法:20例经病理确诊的鼻咽癌患者在治疗前、治疗中、治疗结束分别行99mTc-TF SPECT肿瘤显像,比较各个阶段的早期显像摄取指数(MUI)和延迟显像摄取指数(DUI),同时计算滞留率(RI)。结果:17例患者完成所有的SPECT显像并获得显像参数,有3例患者只完成部分SPECT显像。治疗前的MUI为7.86±2.89,DUI为9.50±3.02;治疗中期MUI为5.92±1.55,DUI为7.29±2.11;治疗结束后MUI为5.01±1.95,DUI为6.11±2.70。同一时期显像的MUI与DUI、治疗中期MUI和DUI与治疗前相比以及治疗结束后的MUI和DUI与治疗前相比,差异均有统计学意义(P<0.01)。结论:99mTc-TF可用于鼻咽癌患者SPECT肿瘤显像,并在早期即可评价治疗效果,为优化患者治疗方案及个体化治疗提供了帮助,可以作为鼻咽癌早期疗效评价的重要辅助手段。

Objective: To observe the feasibility of 99mTc-tetrofosmin(99mTc-TF)single-photon emission computed tomography(SPECT) tomography for therapeutic response to nasopharyngeal carcinoma(NPC).Methods: 20 patients with NPC from Yancheng NO.1 People's Hospital accepted 99mTc-TF SPECT tomography in the stages of before, during and after treatment. All of the patients were confirmed by biopsy pathology examination. The imaging early uptake index(MUI) of each stage were compared and the delayed imaging uptake index(DUI), the retention rate(RI) were also calculated.Results: 17 patients completed all of the SPECT imaging and the imaging parameters were obtained, three patients completed part of SPECT imaging. The MUI of pre-treatment was 7.86±2.89, and DUI was 9.50±3.02; the MUI of under-treatment was 5.92±1.55, and DUI was 7.29±2.11; the MUI of post-treatment was 5.01±1.95, and DUI was 6.11±2.70. The difference between MUI and DUI in the same period imaging was statistically significant(P<0.01). The difference of MUI and DUI in pre-treatment imaging compared with that of under-reatment showed statistically significant(P<0.01). The difference of MUI and DUI between post-treatment imaging and pre-treatment imaging was also statistically significant(P<0.01). But there was no statistically significant difference between post-treatment imaging and under-treatment imaging(P>0.05).Conclusion: 99mTc-TF can be used for SPECT tumor imaging of patients with NPC, and the 99mTc-TF SPECT imaging helps to evaluate the efficacy of the treatment in the early stage, optimal treatment scheme for patients, and provide individualized treatment. It can be used as an important auxiliary mean in early curative effect evaluation of NPC.
