磁共振ADC值结合磁共振波谱分析在复杂部分性癫痫早期诊断中的应用价值分析

摘要:

目的:对复杂部分性癫痫患者进行磁共振ADC值结合磁共振波谱分析(MRS),同时进行脑电图检查,分析磁共振ADC值、MRS与脑电图特征的相关性,探讨磁共振ADC值结合MRS在复杂部分性癫痫早期诊断中的应用价值。方法:选择在我院进行诊治的复杂部分性癫痫患者55例作为观察组,同期选择在我院体检的健康者55例作为对照组,两组都进行磁共振ADC值结合1H-MRS测定,同时进行脑电图检查与相关性分析。结果:对照组两侧海马平均ADC值为(0.89±0.14)×10-3mm2/s,观察组患侧为(1.14±0.28)×10-3mm2/s,对侧为(0.92±0.24)×10-3mm2/s,观察组患侧ADC值明显高于对照组(t=8.224,P<0.05)。观察组与对照组丘脑代谢物相比有明显差异(P<0.05),同一组的丘脑右侧、左侧代谢物对比无明显差异(P>0.05)。对照组脑电图都表现为背景正常,观察组脑电图表现为背景正常18例,慢化32例,快活动50例,棘慢波40例。直线相关性分析显示复杂部分性癫痫的脑电图背景特征(慢化、快活动、棘慢波)都与ADC、NAA/Cr-PCr、NAA/(Cr-PCr+Cho)、Cho/Cr-Per都有明显的相关性(P<0.05)。结论:磁共振ADC值结合MRS在复杂部分性癫痫早期诊断中的应用能有效反映病情状况,且与患者脑电图特征明显相关,有很好的应用价值。

Objective: Toevaluate the value of magnetic resonance ADC combined with magnetic resonance spectroscopy (MRS) in the early diagnosis of complex partial epilepsy.Methods: 55 patients of complex partial epilepsy in our hospital for diagnosis and treatment were selected as the observation group,and the 55 healthy cases in our hospital physical examination were selected as the control group at the same period,two groups were given the ADC combined with 1H-MRS examination,EEG were carried out in the same period its correlation with ADC and 1H-MRS.Results: The both sides ADC value of the hippocampus in the control group was (0.89±0.14)×10-3mm2/s,the ipsilateral in the observation group was (1.14±0.28)×10-3mm2/s,meanwhile the contralateral in the observation group was (0.92±0.24)×10-3mm2/s.ADC value of the ipsilateral in the observation group was significantly higher than that in the control group (t=8.224,P<0.05).Compared with the control group,the metabolite of thalamus in the observation group was significantly different (P<0.05).There was no significant difference in the right and left metabolites between the thalamus and the thalamus in the same group (P>0.05).The control group showed normal background.EEG of the observation group was normal in 18 cases,slowing down in 32 cases,fast moving in 50 cases,andspike and ware wave in 40 cases.The linear correlation analysis showed that the EEG background characteristics of complex partial epilepsy were significantly related to ADC,NAA/Cr-PCr,NAA/(Cr-PCr+Cho) and Cho/Cr-Per (P<0.05).Conclusion: The combination of magnetic resonance ADC and MRS in the early diagnosis of complex partial epilepsy can effectively reflect the condition of the disease,which is significantly related to the EEG characteristics of the patients and has a very good application values.
