交锁髓内钉治疗成人股骨干骨折的疗效研究

摘要：

目的：分别采用锁定加压钢板螺钉内固定及交锁髓内钉内固定治疗成人股骨干骨折，比较围手术期指标、膝关节功能及并发症，探讨交锁髓内钉治疗成人股骨干骨折的疗效。方法：2011年10月至2017年1月选择在我院骨科住院诊治的80例成人股骨干骨折患者当做研究对象，依据不同的手术方法将80例患者均分为对照组和观察组，对照组采用锁定加压钢板螺钉内固定治疗，观察组采用交锁髓内钉内固定治疗，观察围手术期指标、膝关节功能及并发症。结果：两组手术时间、住院时间对比无明显差异，在骨痂出现时间、骨折愈合时间及术中出血量方面，观察组比对照组明显要少（P<0.05）。观察组术后3个月的膝关节功能优良率为87.5%，对照组为65.0%，观察组高于对照组（P<0.05）。对照组术后3个月的静脉血栓形成、切口感染、髋内翻等并发症发生率为25.0%，而观察组为10.0%，比对照组明显要少（P<0.05）。结论：交锁髓内钉治疗成人股骨干骨折能减少创伤，促进骨折愈合，减少术后并发症的发生，促进膝关节功能的恢复。

Objective: To evaluate the effects of interlocking intramedullary nail in treating of adult femoral shaft fracture.Methods: From October 2011 to January 2017, 80 patients with adult femoral shaft fracture in our hospital were selected as the research objects, All the patients were equally divided into control group and observation group accorded to different surgical methods, the control group was given the locking compression plate fixation. The indexes of perioperative period, the function of the knee joint and the complications were observed.Results: There were no significant differences of operation time and hospitalization time between the two groups. The amount of bleeding, the time of callus formation and the healing time of the fracture in the observation group were significantly less than those in the control group(P<0.05). The postoperative 3 month of excellent rate of knee joint function was 87.5% in the observation group, 65.0% in the control group, with a statistic difference(P<0.05). The postoperative3-month of complication rate of venous thrombosis,incision infection and coxa varus in the control group was 25.0%, it was 10.0% in the observation group, the difference being significant(P<0.05).Conclusion: Interlocking intramedullary nailing for adult femoral shaft fractures can reduce trauma, promote fracture healing, reduce postoperative complications, and promote the recovery of knee function.
