乳腺囊内乳头状癌DR钼靶的误诊原因分析

摘要：

目的：探讨乳腺囊内乳头状癌（IPC）DR钼靶摄影的误诊原因，以提高对此病的诊断水平。方法：回顾性分析接受乳腺钼靶检查并经手术病理组织检查及彩超证实为误诊15例患者的影像资料。结果：IPC误诊为乳腺导管内乳头状瘤5例，乳腺纤维腺瘤为2例，乳腺囊肿为2例，炎症为2例，乳腺导管内乳头状瘤、乳腺纤维腺瘤、乳腺囊肿误诊为IPC为4例。结论：通过IPC误诊病例分析其原因，以便在诊断此类疾病吸取教训，更应该结合患者临床症状、触诊包块性质，根据乳腺钼靶片的直接征象以及间接征象，应用DR图像后处理技术，结合彩超、病理组织检查及分泌物实验室检查以进一步降低误诊率。

Objective: To investigate the causes of misdiagnosis of DR mammography in breast intraductal papillary carcinoma (IPC) so as to improve the diagnosis of this disease. Methods: The mammographic data of 15 patients who underwent mammography were retrospectively analyzed and proved by operation, pathological examination and color Doppler ultrasound.Results: 5 cases of IPC were misdiagnosed as intraductal papilloma, 2 cases as breast fibroadenoma, 2 cases as breast cyst, 2 cases as inflammation. 4 cases of Intraductal papilloma, breast fibroadenoma, breast cyst were misdiagnosed as IPC. Conclusion: Through the analysis of its causes of misdiagnosis of IPC, lessons were drawn in the diagnosis of these diseases. We should combined clinical symptoms with palpation of masses, paying attention to the direct signs of breast mammography and indirect signs, as well as combine postprocessing technique with the application of color Doppler DR images, pathological examination and laboratory examination of secretions to further reduce the misdiagnosis rate.
