心脏术后患者容量反应性评估的研究进展

摘要:

心脏术后患者容量反应性评估对指导液体治疗至关重要。目前容量反应性评估包括常见的中心静脉压检测、肺动脉漂浮导管、脉波指示剂连续心排血量监测、每搏变异度、脉压变异率和心脏超声等检查方法,以上方法对容量反应性的评估均有一定参考价值但准确性偏低,需要综合分析后做出判断。血流动力学食道超声因其可以持续获得动态心脏超声影像等优点,推断其对患者的容量反应性的评估有较高准确性,应用价值值得期待。

Evaluation of volume responsiveness in patients with postcardiac surgery is essential for guiding fluid therapy. The common evaluation measures of volume responsiveness, including central venous pressure, pulmonery artery catheter, pulse indicator continous cardiac output, stroke volume variation, pulse pressure variation and trans-thoracic or transesophageal echocardiography, had certain reference value for the evaluation of volume responsiveness but low accuracies, and we need to make a comprehensive analysis to pass judgment. Hemodynamic-transesophageal echocardiography might have a high accuracy in the evaluation of volume responsiveness for its continuously acquiring dynamic cardiac ultrasound image, and we look forward to its broad scope in clinical application value.
