CÞ与MRI对不同类型外伤性寰枢椎骨折的诊断价值

摘要:

目的:通过CÞ检查结果和MRI检查结果的对比分析,探讨二者在不同受力机制造成的外伤性寰枢椎骨折中的临床应用价值。材料与方法:回顾我院70例高度怀疑外伤性寰枢椎骨折患者的CÞ及MRI影像资料,评估CÞ与MRI检查对外伤性寰枢椎骨折中不同骨折类型的检出结果及显示情况。结果:在70例患者中CÞ发现寰枢椎骨折6¹例,占87.¹4%。其中屈曲型³4例,55.74%;伸展型9例,¹4.75%;垂直型¹²例,¹9.67%;旋转侧屈型6例,9.8³%。MRI发现寰枢椎骨折58例,占8².86%。其中屈曲型³6例,6².07%;伸展型³例,5.¹7%;垂直型¹¹例,¹8.97%;旋转侧屈型8例,¹³.79%。对于外伤性寰枢椎骨折的总体检出率CÞ高于MRI,但对于不同损伤类型的寰枢椎骨折CÞ与MRI存在着较大的差异。伸展型骨折CÞ的检出率明显高于MRI,屈曲型骨折和旋转侧屈型骨折MRI的检出率略高于CÞ,垂直型骨折CÞ与MRI的检出率并无明显差异。结论:对于不同受力机制的外伤性寰枢椎骨折CÞ与MRI的检出率存在着较大的差异。伸展型骨折CÞ的检出率明显高于MRI,屈曲型骨折和旋转侧屈型骨折MRI的检出率略高于CÞ,垂直型骨折CÞ与MRI的检出率并无明显差异。
