超声内镜联合自体荧光内镜在早期食管癌中的诊断价值

摘要:

目的:分析超声内镜联合自体荧光内镜在早期食管癌中的诊断价值。方法:回顾性分析2013年1月-2015年11月本院收治的30例经普通白光内镜诊断为食管黏膜病变患者行超声内镜联合自体荧光内镜检查的临床资料,并与病变组织病理学检查结果进行对比,以分析两种检查方式联合对早期食管癌的诊断价值。结果:本次30例患者经手术病理学确诊早期食管癌9例;采用超声内镜联合自体荧光内镜检查30例食管黏膜病变患者,根据超声层次与荧光共诊断癌性病变9例,经病理学结果验证8例为癌性病变,1例为非癌性病变;两者联合诊断为非癌性病变21例,经病理学结果验证1例为癌性病变,20例为非癌性病变。超声内镜联合自体荧光内镜检查诊断早期食管癌准确度、敏感度、特异度分别为88.89%(8/9)、88.89%(8/9)、95.24%(20/21)。结论:超声内镜联合自体荧光内镜可提高食管病变诊断率,对早期食管癌病变具有较高诊断准确率,值得临床应用。

This study was aimed at asses the value of Endoscopic ultrasonography(EUS)In combination with Autofluorescence Imaging(AFI) in the diagnosis of earlyesophageal carcinoma.Methods A retrospective analysis of 30 patients with Esophageal mucosal lesions was performed, who underwent Endoscopic ultrasonography(EUS) in combination with Autofluorescence Imaging(AFI) treatment in our hospital from January 2013 to November 2015,and compared with histopathological results,To analyze the diagnostic value of the combination treatment. Resuits In 30 cases of patients, 9 cases were diagnosed with early esophageal carcinoma by operative pathology ;In 30 cases of patients with esophageal mucosal lesions, 9 cases were diagnosed with cancerous lesions by Endoscopic ultrasonography(EUS) combined with Autofluorescence Imaging(AFI)treatment, and in which, 8 cases of cancerous lesions and 1 case of non cancerous lesions by pathological results verification; 21 cases were diagnosed with non cancerous lesions by combined diagnosis, and in which, 1 case of cancerous lesions and 20 cases of non cancerous lesions by pathological results verification; Accuracy, sensitivity and specificity in diagnosis of early esophageal cancer by Endoscopic ultrasonography(EUS) combined with Autofluorescence Imaging(AFI)treatment are respectively: 88.89%(8/9)、88.89%(8/9)、95.24%(20/21)。   Conclusion Endoscopic ultrasonography(EUS) combined with Autofluorescence Imaging(AFI)treatment can improve the rate of esophageal lesions diagnostic,and enhance the diagnostic accuracy for early esophageal lesions ,thus it is valuable of clinic application.
