螺旋CT引导下氩氦靶向治疗恶性实体肿瘤的疗效观察

摘要：

目的通过临床试验研究，观察螺旋CT引导下氩氦靶向治疗恶性实体肿瘤的疗效及其影响因素，为恶性实体瘤患者的临床治疗方案选择提供依据。方法选择150例经病理诊断证实为恶性实体肿瘤的患者，在螺旋CT引导下进行氩氦刀冷冻消融治疗，分析瘤体冰球覆盖率，并在术后1、3、6、12个月分别开展随访监测，记录疗效及并发症发生情况。结果 150例患者共有瘤体265个，瘤体直径范围为1.2cm~12cm，中位数为4.21cm。与直径大于3cm的瘤体比较，瘤体小于3cm的冰球覆盖率较好（P<0.05）。术后1个月，3个月，6个月，12个月的肿瘤消融治疗有效率分别为81.13%、71.76%、71.07%和68.10%；肿瘤控制率分别为96.23%、91.37%、85.54%和80.95%；肿瘤直径和肿瘤分期是术后一年疗效的影响因素（P<0.05）结论螺旋CT引导下氩氦靶治疗恶性实体肿瘤的效果较好，可以广泛应用。

Objective To explore and observe the clinical treatment effect of spiral CT guidance argon-helium cryablation on malignant solid tumors by applying clinical trial study, and to explore the influence factors of the treatment effect. Methods 150 patients of malignant solid tumors were selected who were confirmed by pathology. After the cryablation therapy, follow-up were carried in 1, 3, 6, 12 months respectively, and treatment effect and related complications were recorded. Results There were 265 tumors in the 150 patients. The diameter of tumors ranged from 1.2 cm to 12 cm, with the median of 4.21cm. Compared with the ice coverage of tumors larger than 3 cm in diameter, ice coverage of tumors less than 3 cm was better (P<0.05). The efficient rates were 81.13%, 71.76%, 71.07% and 68.10% in 1, 3, 6, 12 months respectively, and the control rates were 96.23%, 91.37%, 85.54% and 80.95%, respectively. Tumor diameter and tumor staging were the influence factors of curative effect in one year after therapy(P＜0.05). Conclusion spiral CT guidance argon-helium cryablation was good for the treatment of malignant solid tumors, which could be widely used.
