下鼻甲射频消融术后迟发性鼻出血7例分析

摘要：

目的探讨下鼻甲低温等离子射频消融术后迟发性鼻出血的原因及防治对策。方法回顾我院自2 0 0 8年 6月—2 0 13年1月共发生7例下鼻甲低温等离子射频消融术后迟发性鼻出血患者的临床资料，总结分析导致迟发性鼻出血的主要原因、临床处理方法、处理效果以及防治对策。结果造成下鼻甲射频消融术后迟发性鼻出血的主要原因是手术创面感染，与此相关的因素包括患者自身因素（高血压、贫血等）、术中消毒不严格、操作不当及术后健康教育不到位及手术创面处理不当等。治疗方法主要是行前鼻孔填塞和抗感染治疗，必要时行鼻内镜下双极电凝止血术。经治疗，7例患者全部治愈，未发生再次出血。结论全面的术前分析和采取预防措施、准确的术中和术后处理及详细的出院指导是预防下鼻甲低温等离子射频消融术后迟发性鼻出血的关键。

Objective: To study the cause and treatment of delayed bleeding after endoscope in plasm radiofrequency at low temperature with inferior nasal concha hypertrophy. Methods: Records of 7 cases with delayed bleeding after endoscope in plasm radiofrequency at low temperature with inferior nasal concha hypertrophy. That were operated on our department between 2008-2013 was retrospectively analysed. Results: Bacterial infection was the major cause of delayed bleeding after the operation. Anterior nares packing combine anti-infective treatment was the basic method of treatment. Bi-polar electric coagulation was done if necessary. Conclusion: The therapeutic success rate of delayed bleeding after endoscope in plasm radiofrequency will be boosted by taking protective and therapeutic measures pre-and intra-operatively.
