摘要：

目的探讨下颌升支截骨术不同术式矫治下颌畸形的效果。方法选择我院口腔颌外科43例下颌前突畸形患者，并随机分为SSRO组(25例)和IVRO组(18例) ，比较分析两组患者的牙合力、咀嚼效能以及髁突相对位置变化。结果 SSRO组牙合力术后3个月、术后6个月分别为(11.35±4.78)kg、(16.85±2.59)kg,IVRO组术后3个月、术后6个月分别为(7.64±5.12)kg、(14.42±3.12)kg ，两组差异具有统计学意义(P均<0.05)；SSRO组咀嚼效能术后3个月、术后6个月分别为(1.05±0.36)、(1.54±0.22),IVRO组术后3个月、术后6个月分别为（0.78±0.38）、(1.35±0.31)，两组差异具有统计学意义(P均<0.05)；SSRO组术后髁突向前、向后移位分别占16.0%、84.0%，IVRO组分别占44.4%、55.6%，两组差异具有统计学意义(P均<0.05)。结论SSRO术式矫治下颌前突畸形在牙合力、咀嚼效能、髁突位置改变均优于IVRO术式，且其对颜面侧方影响小，值得临床推广应用。

Objective To investigate effect of mandibular prognathism treated by SSRO and IVRO. Methods we selected 43 cases patients with mandibular prognathism who were randomly divided into SSRO group (n = 25 ) and IVRO group ( n = 18).Bite force, masticatory efficiency and relative position changes of patients of all two groups were comparativly analysis.Results The bite force in 3 months, 6 months after operation of SSRO group were respectively ( 11.35 ±4.78) kg, ( 16.85 ±2.59) kg;and SSRO group were respectively ( 7.64 ±5.12) kg, ( 14.42 ±3.12) kg，the difference between the two groups was statistical significance ( P all <0.05).The chewing efficiency in 3 months, 6 months after operation of SSRO group were respectively ( 1.05 ±0.36), ( 1.54 ±0.22);and SSRO group were respectively (0.78 ±0.38), (1.35 ±0.31), the difference between the two groups was significant (all P <0.05 ).The condyle forward, backward displacement of SSRO group were respectively 16%, 84%；and IVRO group were 44.4%, 55.6%，the difference between the two groups was significant (all P <0.05).Conclusion Mandibular prognathism treated by SSRO compare with IVRO.bite force, masticatory efficiency, condylar positional changes are better. and SSRO influence on facial lateral is little, it is worthy of clinical application.
