[Abstract] Objective To explore how to improve the tumor(carcinoma in cardia-fundus especially involved lower esophagus within 1~2cm and upper stomatch cancer) resection rate by transabdominal gastrectomy and reduce the surgical trauma, combined with comprehensive treatment for increasing therapeutic effect. Methods Of the 61 patients, 29cases underwent the HSAPC, intraperitoneal chemotherapy(IPC) and other therapies during operations. Results The treatment group(29cases): 28cases (all by transabdominal surgery)underwent radical resection while 3cases received palliative resection and 1case underwent transthoracic surgery, 93.1percent of patients were alive a year later, 62.1percent of patients at 3years, 48.2percent at 5years and 20.7percent at 10years; The control group (32cases): 19cases underwent transabdominal surgery underwent radical resection while 7cases received palliative resection and 13case underwent transthoracic surgery. 81.2percent of patients were alive a year later, 40.6percent of patients at 3years, 25percent at 5years and 9.4percent at 10years. Conclusion Preoperative HSAPC can significantly improve the tumor resection rate and increase the postoperative survival time of the patients with carcinoma in cardia-fundus partly involved lower esophagus and upper stomatch, the combined therapy may reduce recurrence. |
