异丙酚对大鼠肺缺血再灌注损伤保护作用的实验研究

[摘要] 目的: 通过建立大鼠肺缺血再灌注模型, 观察异丙酚(Propofol)对大鼠缺血再灌注损伤(Ischemia Reperfusion,IR)肺的保护作用,并探讨其作用机制。方法:随机将30只雄性SD大鼠分为3组(每组10只):假手术组(A)(左侧开胸后不阻断肺门);肺缺血再灌注组(B)(左侧肺门阻断45min再灌注120min),异丙酚组(C)(左肺门钳闭前30 min时给予股静脉注射异丙酚5 mg/kg,继用微量泵以50 mg/kg/h的速度持续静脉输注,左侧肺门阻断45min再灌注120min)。B组用相同的方法处理,但静脉输入等量的生理盐水;三组在开胸前均给予气管插管,机械通气。再灌注结束时抽取左心室血检测动脉血氧分压(PaO2),获取左肺组织,光镜下观察左肺组织和细胞损伤,计算左肺湿干重比,评价肺水肿程度,制作肺组织匀浆并检测其中丙二醛(Malondialdehyde,MDA)含量、肿瘤坏死因子-α(Tumor necrosis factor-α, TNF-α)、白细胞介素-6(interleukin-6;IL-6)和超氧化物歧化酶(superoxide dismutase,SOD)活性。结果:IR组、异丙酚组与假手术组相比,大鼠血氧分压明显下降(P<0.05),肺组织中的MDA、TNF-α、IL-6含量及肺W/D值明显增加(P<0.05),SOD活性减弱(P<0.05);异丙酚组与IR组相比,大鼠的血氧分压升高(P<0.05),肺组织中的MDA、TNF-α、IL-6含量及肺W/D值下降(P<0.05),SOD活性加强(P<0.05)。肺组织病理切片示假手术组损伤轻微,IR组损伤严重,异丙酚组损伤重于假手术组,轻于IR组。结论:1、缺血再灌注对组织和细胞的损伤严重;2、异丙酚对缺血再灌注损伤的SD大鼠肺组织有一定的保护作用,可能与异丙酚能清除氧自由基,抑制炎症因子产生有关。

[Abstract] Objective : To observe the protective effect of Propofol in rats by establishing models of lung ischemia-reperfusion.Methods:Thirty male Sprague-Dawley rats were randomly divided into three groups of ten each: sham group(A) (the left chest was opened without clamping the left hilum of lung),Lung ischemia reperfusion injury group(B)(the injury was induced by clamping the left lung hilum for 45min followed by 120min reperfusion), Propofol group(C)(Propofol<5mg/kg> was injected though femoral vein 30min before clamping left lung hilum, then propofol<50mg/kg/h>was injected with the trace pump. Left lung hilum was clamped for 45min and followed by 120min reperfusion). Group B use the same method to deal with, but inject the same amount of saline; Three groups were given tracheal intubation、mechanical ventilation before opening chest. At the end of the reperfusion,the blood sample was withdrawn from the left ventricals to test the PaO2,the left lung was harvest.We can observe the injury of issue and cell by light microscope,to evaluat pulmonary edema through lung wet-to-dray (W/D) ratio, Malondialdehyde(MDA), Tumor necrosis factor-α(TNF-α)、interleukin-6(IL-6) content and Superoxide dismutase(SOD) activity in lung tissues were measured. Results: The W/D ratio, the lung MDA、TNF-α、IL-6 content in group B and C were significant higher than those in group A,while the PaO2 and SOD activity in group B and C were much lower than those in group A(P<0.05).As compared with group B, The W/D ratio,the lung MDA、TNF-α、IL-6 content in group C were decreased, while the PaO2 and SOD activity in group C were increased notably(P<0.05).Lung histopathology shows: group A slight damage, group B baldly damaged, group C damaged lighter than group B.Conclusion:1、Ischemia reperfusion can make tissue injury seriously.2、Propofol can prevent SD rats lung Ischemia reperfusion injury to some extent by removing oxygen free radicals and inhabiting inflammatory factor.
