子宫苗勒腺肉瘤PTEN、Survivin的表达及其与肿瘤临床病理的关系

摘要：

目的：探讨子宫苗勒腺肉瘤（UMA）PTEN和Survivin蛋白的表达水平，分析其与UMA的临床病理关系。方法：选取2010年5月至2015年5月亳州市人民医院收治的7例UMA患者，正常子宫内膜者10例；手术常规采集患者病变部位进行石蜡切片的制备及免疫组化检测PTEN、Survivin蛋白的阳性表达率；等级相关分析法分析PTEN与Survivin蛋白的相关性。结果：7例UMA中子宫内膜腺肉瘤5例，宫颈腺肉瘤2例，病变部位可见良性或非良性的上皮混合肉瘤间质构成的肿瘤，术后1例患者复发且死亡，其余患者均恢复良好；免疫组化结果显示，UMA患者抑癌蛋白PTEN阳性表达2例，而其在正常组子宫内膜中均阳性表达；UMA患者抑制凋亡蛋白Survivin表达6例，而其在正常组子宫内膜中阳性表达1例；UMA PTEN、Survivin蛋白表达在组织学分级、淋巴转移、肌层浸润中差异均无统计学意义（P>0.05），与手术病理分期中差异具有统计学意义（P<0.05）。Survivin与PTEN在UMA中的表达呈显著负相关（r=-0.602，P=0.012）。结论：UMA患者PTEN显著低表达，Survivin显著高表达，两者均与UMA的发生发展相关，在UMA中具有重要意义。

Objective: To investigate the expressions of PTEN and Survivin proteins in uterine Mullerian adenosarcoma (UMA), and to analyze their relationships with clinicopathological of UMA. Methods: From May 2010 to May 2015, seven patients with UMA and ten patients with normal endometrium were selected; paraffin sections of lesions were prepared by routine collection and immunohistochemical experiment was performed to detect the positive expressions of PTEN and Survivin proteins in the lesions; the correlation between PTEN and Survivin proteins was analyzed by rank correlation analysis.Results: The results showed five cases of endometrial adenosarcoma and two cases of cervical adenosarcoma, benign or non benign epithelial mixed sarcoma tumors were seen in the lesion, after operation, one patient relapsed and died, and the rest of the patients recovered well; the results of immunohistochemistry showed PTEN was positive in two UMA patients, but it was positive in all normal endometrium; Survivin was postive in six UMA patients, but it was positive in one normal case's endometrium; the expressions of PTEN and Survivin in endometrium had no significant difference in histological grade, lymphatic metastasis and myometrial invasion (P>0.05), but there was significant difference insurgical pathological stage (P<0.05). There was a significant negative correlation between Survivin and PTEN expression in UMA (r=-0.602, P=0.012).Conclusion: PTEN is significantly lowly expressed while Survivin is significantly highly expressed in UMA patients, they are related to the occurrence and development of UMA, which is of great significance in UMA.
